Community Needs Assessment
T.J. Health Columbia continues to research the most effective ways to best serve the needs of the community in which it serves. Over the past two years, the organization has participated in conducting a Community Health Improvement Plan in partnership with the Lake Cumberland District Health Department/Adair County W.A.T.C.H (Working Actively Toward Community Health) Coalition. The Lake Cumberland Community Health Improvement Plan is a result of the MAPP process and determines the areas of greatest health need for each county. The plans are driven by all the community health coalition partners to help improve the health status of a county within the region. Below you will find a copy of the Community Health Improvement Plan and the Executive Summary published by the Lake Cumberland District Health Department/W.A.T.C.H Coalition. Also, shown below is T.J. Health Columbia’s Community Health Needs Assessment and Strategic Implementation Plan, that demonstrates how the hospital plans to help meet the needs identified within the community assessment. The conducting of the Community Health Needs Assessment and the implementation of the Strategic Plan are both in an effort to comply with the Affordable Care Act of 2010 and to maintain organizational nonprofit status.
